Mounjaro Weight-Loss Plateau: What to Do When Weight Stops Falling, and Should You Increase the Dose?

A weight-loss plateau while using tirzepatide is common and does not automatically mean the medicine has stopped working. Before increasing the dose, distinguish a true long-term plateau from normal weekly variation, constipation, fluid shifts or a transition into weight maintenance.

What does “weight plateau” mean in research?

The SURMOUNT plateau analysis did not define plateau as “the scale did not move this week.” It used repeated longer intervals. This matters because short-term body weight can change with salt, carbohydrate, bowel contents, menstruation, travel and exercise.

Plateau does not necessarily mean loss of drug effect

As body weight falls, energy requirements also decline. A calorie deficit that produced rapid loss early in treatment may become smaller later. The medication may still be helping appetite regulation and preventing regain even when the scale is no longer dropping quickly.

Seven things to check before increasing the dose

  1. Use 7-day average weight, not a single reading.
  2. Check constipation.
  3. Review drinks, alcohol and snacks.
  4. Check whether daily steps/activity fell.
  5. Review sleep and stress.
  6. Measure waist circumference.
  7. Ask whether you are already close to a healthy treatment goal.

When can escalation be reasonable?

Escalation may be reasonable after the current dose has been used for at least the labeled interval, the longer-term trend is truly insufficient for the treatment goal, and side effects, nutrition and hydration are acceptable. It is not reasonable simply to “break a plateau” with a dose increase regardless of tolerability.

What if you are already on 15 mg?

There is no higher labeled Mounjaro dose. At 15 mg, focus shifts to maintenance, food quality, activity, constipation, sleep, other weight-promoting medications and whether further weight loss is medically needed. See the 15 mg plateau guide.

Should you stop treatment to “reset” it?

There is no established “reset” strategy. In SURMOUNT-4, participants who switched from tirzepatide to placebo after substantial weight loss regained an average 14.0% from week 36 to week 88, whereas those who continued lost another 5.5% on average. Do not stop and restart on your own.

Frequently asked questions

Is a Mounjaro plateau normal?

Yes. Long-term trials show that many people eventually reach a weight plateau; it does not automatically mean treatment has failed.

Should I increase the dose after one week without weight loss?

Usually not based on one week alone. Review a longer trend and other causes of short-term scale variation.

Can 15 mg also plateau?

Yes. 15 mg is the maximum labeled dose, but long-term weight loss still tends to slow and stabilize.

Should I stop Mounjaro for a while to make it work again?

There is no established reset method, and withdrawal trials show substantial average weight regain after stopping tirzepatide.
