7.5 mg is not always only a transition dose?

Although 7.5 mg is often used while titrating from 5 mg to 10 mg, a patient may remain on 7.5 mg when it provides adequate benefit and is tolerated. The goal is not to reach the highest possible dose, but to use the lowest dose that provides sufficient benefit.

Long-term monitoring?

Weight and waist trend, appetite, glucose and HbA1c when applicable. Nausea, vomiting, constipation, gallbladder symptoms, hydration, kidney function risk, nutrition, and muscle strength. Pregnancy plans, surgery, new medications, and changing health conditions.

Continue, increase, reduce, or stop?

Continue when effective and tolerated. Increase only when control is inadequate and adverse effects are acceptable. Reduce when adverse effects or inadequate nutrition appear. Stopping requires a plan because weight regain is common after withdrawal.
